Community Wellness Survey

The Fresno County Suicide Prevention Collaborative is a group of local government agencies, schools, healthcare, public safety, and community organizations that are working together to develop a suicide prevention plan for the county. We need your help to build the best plan possible by learning from the experiences, ideas, and opinions that you share in this survey. The information that you share will be used along with other data to describe the challenges and strengths facing Fresno County suicide prevention efforts and will be compared to future public opinion surveys to see how things change as we use different prevention strategies.

Your participation is voluntary, and you can decide to skip any questions that you don't want to answer. Your survey responses will be anonymous.

The survey takes between 15 - 40 minutes to complete, depending on which questions you choose to display and answer.

Prevention Communities, internationally recognized in suicide prevention, is working with a group of consultants under contract with Fresno County Department of Behavioral Health to provide technical assistance to the Collaborative.
